The consideration for this sale shall be the sum of _________ plus the terms and conditions hereinafter set forth.

Your licensed veterinarian will examine this puppy within three days of receipt for a wellness check. You will set up a puppy plan schedule for vaccinations including Rabies and the 9-, 12-, and 16-week DHPP vaccinations, as well as heartworm, flea, and tick preventatives. 

The buyer agrees that: 

The puppy/dog mentioned above will be given adequate and proper veterinary care throughout its lifetime and all inoculations will be maintained to ensure preventative health care. That said puppy/dog will be given adequate and proper food, water, and housing as well as properly exercised. This dog shall not be neglected or mistreated. The dog cannot be tied or chained for the purpose of being kept outside. That this dog will not be housed outside. 

Buyer agrees that this puppy will only be registered with the American Kennel Club (AKC). 

The Buyer agrees to return said dog to the Seller if the Buyer can no longer provide an appropriate home as described above, this dog should never be surrendered to a shelter. The purchase price will not be refunded however, the seller promises to find/provide an appropriate home as described above. 

That buyer agrees to the following: Your licensed veterinarian will examine this puppy within three days of receipt for a wellness check. In the unlikely event that you have any health concerns, all concerns will be addressed immediately following the veterinary visit, and the puppy will be returned to the breeder for a full refund. The buyer understands that for the welfare of the puppy and the breed; this puppy is being sold with Limited Registration which means as pet quality and they are to be spayed or neutered after one year of age. It is agreed that said dog will not be used for breeding without Full Registration and understood by the buyer that if this dog is used for breeding, having a litter or siring a litter, this contract is null and void. It is understood that said puppy will have an AKC limited registration, meaning that your dog is AKC registered however no offspring from said dog is eligible for AKC registration of any kind. Full Registration rights will be considered to experienced show homes or to experienced breeders only and are subject to approval. 

The Buyer has read and understands the foregoing and agrees that all parts and portions therefore constitute a binding agreement.
